Medical Clearance Process

Step 1: Complete the Dive Medical Questionnaire

All prospective divers are required to fill out a standard dive medical questionnaire. This form helps identify any medical conditions that could affect your safety while scuba diving.

Step 2: Review Your Results
  • If you answer “No” to all medical questions, you are typically cleared to begin training.

  • If you answer “Yes” to any question, a physician’s approval is required before participating in scuba activities.

Step 3: Schedule a Dive Medical Screening (If Needed)

If medical clearance is required, we recommend scheduling a Dive Medical Screening with Dr. Murray.

Dr. Murray will evaluate your health, discuss any potential risks, and determine whether scuba diving is safe for you.
